I successfully insured and registered my 2010 Gio Rebel on my first try at the local Insurance/Registry place.
I took in the NVIS and Certificate of orgin that I got with my ATV (I filled out any blanks) plus I used this template that I found on the net to make a bill of sale. After filling any blanks and tossing on some signitures on I had no problem at all. Plates + Reg in under 30 mins.
This is the bill of sale I used, I filed in everything with info from my online invoice. I edited out my personal info because I don't know how legal this was. I also signed my name near the top where my name and address is since they require the signiture of seller and buyer here.

I used Open Office 3.0 to edit all the info, printed it off, signed it and done.
