Front sprocket lock stripped again!!

It keeps happening!!

Chain aligment and tension are as close to perfection as possible, don't know what else to do

Can this be caused by a bad output shaft?

It gets stripped from the front.

Sorry, English is not my main language. The side of the sprocket clip that gets grind by the output shaft is the front side, it grinds the clip's teeth and the sprocket comes off.

Here are a few pictures and a video, after close inspection looks like the shaft is worn, that's my last suspect
